Quadricepsplasty and Arthrolysis in Stiff Knee



Abstract
Many people who sustain injuries to the femur or muscles of thigh are handicapped for life by varing degree of limitation of motion of the knee joint flexion such s squatting or kneeling.
Many surgical procedures were reported by Thomson (1994), Judet (1959), van Nes (1962), Nicoll (1963), Hesketh (1963), Jahng (1965), Han (1980, 1986), Daoud (1982) and Yoo (1989).
During the period from March 1987 to April 1991, 21 cases of stiff knee joint were trated surgically in our clinics.
1. 21 cases of stiff knee treated by modified Judet¢¥s technique of quadricepsplasty and arthrolysis.
2. Male patient was 19 and female was 2. Age ranged from 16 years to 63 years.
3. The stiff knee were corrected successfully without release of rectus fe4moris muscle in our cases.
4. The string tie was useful in postoperative care. String tie was applied with the knee in a full flexed position and when it was discarded, knee was extended and quadriceps setting exercise was begun.
5. In 15 cases, plate removal added, in 3 cases, screw and wire removed, in two cases, combined quadricepsplasty and bone surgery, for malunion or nonunion of the femur, which showed good result.
6. The average flexion achieved by operation was 126.6¡Æ, the average flexion gain 69.2¡Æ, while the average extension gain 6.4.¡Æ
7. By Judet criteria, the result of 18 cases were excellent, 2 cases were good and 1 case showed fair result.
